Dr. Sullivan is a participating provider in the following NYS health insurance plans which are accepted for diagnostic audiological services only with referral from your primary care physician. Deductibles and co-pays are due and payable at the time of visit.

Hearing instrument fittings are facilitated through Garden City Hearing Services, LTD, Inc, a non-participating provider.
All hearing instruments are fitted through Garden City Hearing Services, LTD, Inc. GCHS is not a participating provider in ANY health insurance program. GCHS does not accept assignment or limiting charges from any health insurance provider. Health insurance providers typically do NOT allow subsidy for hearing instrument acquisition. Contact your provider for details. A receipted bill will be provided from Garden City Hearing Services, LTD upon satisfactory completion of the hearing instrument trial experience. Subsidy reimbursement, if any for non-participating providers, is then sent from provider directly to the patient.
