Dr. Sullivan is a participating provider in the following NYS health insurance plans which are accepted for diagnostic audiological services with referral from your primary care physician. Deductibles and co-pays are due and payable at the time of visit.

Health insurance providers typically do NOT allow subsidy for hearing instrument acquisition. The following plan(s) are known to provide hearing aid benefits. Contact your provider for details. A receipted bill will be provided upon satisfactory completion of the hearing instrument trial experience. Subsidy reimbursement is then sent from provider directly to the patient.
